On September 30, 2004 the newest version of the Driver Medical Examination Report must be used for all commercial driver physical examinations.

The newest form can most easily be recognized by the lowest threshold blood pressure limit of 140/90. If your forms do not indicate this blood pressure your forms cannot be used after September 29, 2004.
